As marijuana legalization spreads in more and more jurisdictions, the potential for incidental exposure is growing in scale. 

What is Secondhand Marijuana Smoke?

It has many of the same chemical compounds as smoke inhaled first-hand, including THC (tetrahydrocannabinol), the major psychoactive component that causes the effects of cannabis. When someone exhales after taking a hit off of a joint, a measurable amount of THC is going into the surrounding environment. Particles released from the user can be inhaled by those in the immediate vicinity causing THC to enter the bloodstream.

The Realities of Drug Test Detection

Employment drug tests utilize standard cutoffs to differentiate between direct use and passive exposure. Most common urine tests utilize a cut-off level of 50 nanograms per milliliter (ng/mL). When used, confirmation tests utilize an even low cutoff level of 15 ng/mL of the active metabolite called THC-COOH. These thresholds were set specifically to avoid false positives from incidental environmental exposure. Under normal conditions, would not expect short duration exposure, indirect or direct from secondhand marijuana smoke to exceed the detection thresholds. There are still number of important factors that may affect whether secondhand smoke will produce a positive drug test:
  • Exposure context – Non-ventilated or poorly ventilated enclosed space is going to substantially increase the amount of THC.
  • Length of exposure and point of exposure – Sitting next to active smokers for a long duration is going to substantially increase your exposure compared to short interactions or higher distance.
  • Potency of cannabis – Current varieties of cannabis often have a greater ratio of THC compared to some cannabis products in decades past. Furthermore, higher doses of THC are potentially transferable through secondhand smoke.
  • Metabolism – Each person has a different metabolism governing how THC is processed and removed from the body. Variables such as individual body type and compositions, metabolism and excretion rate and health will determine the amount of time to clear absorbed compounds.
  • Testing performed after exposure. Urine screening for THC metabolites may detect a result long after exposure, frequently extending many days after exposure. However, blood test detection time frames are often much shorter.

Research

In one of the most commonly referenced studies, non-smokers spent three hours in a confined space with cannabis smokers. From the study, several of the subjects tested positive in urine screening; however only within the first few hours of smoking and can confirm it was all below confirmation limits. A more recent study tested similar conditions under extreme conditions, including small non-ventilated rooms with multiple active smokers. Non-smokers were able to test positive on standard urine tests within a 24 hour exposure window; however, the test results were only transient and not detected on subsequent tests. 

Practical measures to stop violation of drug testing requirements

Outdoor settings will minimize exposure rates compared to enclosed spaces. In enclosed spaces, if possible, significant ventilation would minimize risk. Perhaps distance from active smokers and limiting time of exposure create the best possible preventative action. For individuals with zero tolerance, for example professional athletes or others subject to drug testing by legal means, it remains prudent to avoid any environmental exposure to cannabis consumption – regardless of ventilation, enclosed or outdoors.
